submit a mission grant proposal.

There forms must be submitted by Oct 31st in an

GULF STATES DISTRICT SCHOLARSHIP GUIDELINES AND APPLICATION

There are four (4) documents that need to be downloaded.

One document is the guideline which outlines what steps

need to be taken to submit the Scholarship Form.

The other three (3) are the checklist, scholarship

application and Pastor's Recommendations.

Completed forms need to be sent to the address below by March 15th:

Please submit the voucher form along with the appropriate

receipts to the District President.

DISTRICT MITES REMITTANCE FORM

When sending in mite offerings, Quarterly, mite boxes or

other offerings, please use this form.

District Mites Remittance

DISTRICT HONORARIUM/MEMORIAL FORM

Please submit the Honorarium/Memorial Form when

submitting honorarium or memorial donations.
